As a virtual call center platform provider, I can tell you that our call center clients with outbound campaigns use and love the predictive dialer functionality.
The predictive dialer is especially useful in Outbound IVR campaigns where the system automatically dials out to the uploaded lead list and delivers a prerecorded message or survey with prompting capability. Specific criteria can be selected to determine whether the message is only given to live people, answering machines, etc.

Most B2B providers do support Auto Dialer traffic but there are usually stipulations. Most providers require dialer customers to hit targets for ASR and ACD and often limit CPS. Due to new US legislation, randomized dialing is now prohibited and punishable by law though predictive dialing is still generally accepted. I have heard great things about Vicidial from a customer of mine in the collections business. Vividial is an open source application and is asterisk based and extremely flexible. This open source software also allows for the user to upload campaigns and can be used with an IVR or live agents depending on the applications of course. One thing to keep in mind is that building inbound routes manually instead of using the GUI can be problematic.

are we not missing the real question? Is cold calling dead now we have the communication tools to reach engaged 'opt in' audiences without interupting them?
Your question is pretty broad - I have extensive experience with both hosted dialers and hardware based dialers in both individual and call center applicatins. Of course, they work. They work better if the dialer campaign gives away free twenty dollar bills than if you are giving away free cow manure. The hosted dialer I use can make 100 concurrent calls and has proved itself especially in the political arena with positive repsonse rates as high as 25%.

Cold calling is definitely not dead, as suggested above. And predictive diallers make enormous sense for certain targetted B2B campaigns, I would use them without hesitation for marketing campaigns, and low value short cycle sales, but I'm not a fan of Major Accounts work being handled this way, and I suspect the clients aren't either.
If you have a decent predictive dialler though, you can schedule call backs with it, block a client to agent, and so on... at which point Major Accounts are no longer a problem, the dialler serves as an aide memoire when linked to CRM. You probably need a short pause pre-dialling for the agent to refresh their memory of the account though.
