An Employee Handbook should have the content pertaining to employee only.
For instance, code of conduct, joining formalities, salary breakup explanation (no calculation), eligibility, Leave, claims, workflows, company policy etc.
Any calculation related to payroll and accounts should not be mentioned in the handbook.
An employee handbook is always a brief of Company's Policy Book and hence the right approach is to refer Company's Policy.

...but generally a lot depends on your particular company and the style you wish to adopt. See if your relatives and friends can give you copies of the handbooks that their employers have issued and use them for inspiration and to adapt them to fit the requirements of your particular needs.
Dress code for men and women, meal/break periods, internet usage, description of all available benefits upon completion of probationary period (if there is one), it should have a Contents page in the front with page numbers so you can easily manuvere to the section you want to read about, what to do if you have a problem with your supervisor (chain of command), overtime, and a welcome page.
