The biggest potential minus is not having the right resources in place (policies, procedures, investigators, training)) to deal with complaints as they arise. No ethics hot line may be better than one that that turns out to be a black hole.
The biggest negative is the potential "lack of feedback" from the complaint. An organization that is committed to establishing an ethics hotline, must develop the ability to address the issue and communicate the outcome.
