Organizational change management is a complex issue and a detailed response to this question would take several posts to answer. I address this topic in detail in my latest book, "Principles for Maturing Your System Development Life Cycle: The Ultimate Guide to the SDLC." In fact, the answer to your question can be found in the free download of Chapter 1 I've made available on my website: http://www.ultimatesdlc.com/. I want to encourage you to download this free resource. You'll find several pages dedicated to organizational change management as well as a framework to manage change provided by IMA Worldwide. IMA Worldwide is the developer of the proprietary Accelerated Implementation Methodology (AIM) for organizational change. The only aspect of organizational change not covered in Chapter 1 is multicultural training, but I don't think that's pertinent in your case. I do cover that topic in a later chapter in a discussion about vendor management and outsourcing.
