I'm not an expert in this, but I've started a few and know a number of others who have as well. The simplest thing to do is use a service to make sure you get everything submitted correctly. I used Quickinc.com. There are a number of others and the cost is not that much more than if you do it yourself. Most of the services also give you a bunch of materials for the other things you need to do (such as put a notice in the newspaper, meeting notices, etc.).
