I love questions like this - we could chat all day. Really glad you're asking, Mack, because in terms of how most people think about VoIP, you're right. If your idea of VoIP is Vonage or Skype, it's a race to the bottom and lots of cheap/free minutes.
There is LOTS of interesting stuff going on with VoIP, but it's really in the area of applications, and how it's used to make other forms of communication or processes more efficient. Good companies to watch in this space include Fonolo, Voxeo, Ifbyphone and Interactive Intelligence. I could name more, but these are pretty good places to start.
Branch to Branch calling is a feature that seems to be making it into all of the systems. This feature uses your internet bandwidth to make calls between branches in your company, without using your voice trunks. A new form of Toll Bypass. It also allows centralized operator positions, centralized voicemail, etc.
How about Automated Assistances, that will automatically forward your calls when you tell your chat program you are away. Those are now reality, and a lot more features with these xmpp bots.
And my favorite - the $6.95 client that runs on your IPHONE as a sip client allowing you to use wifi connections to access your office phone features, and without using your cellular minutes. Traveling to Mexico a lot - save boatloads of money by bypassing their toll system.
