Going to try to answer this without sounding like a sales pitch, since that is what my company does... TEM (telecom expense management) comes in all sizes and from companies ranging from mom/pop shops to corporate giants. Each has their own way and charges accordingly. Most like to push a software package, web based product or even have their employees at your location to manage it. With these types of services, you are getting a "value add" in the form of ease of management, or a software package that simplifies your AP process. Any telecom cost reduction is usually a bonus to help offset the cost of the service.
Others, like my company, offer a cost reduction only approach. Through active invoice management and monthly auditing, we ensure that the client is paying the correct amount for Telecom, and no more. Many (along with my company) perform this on a contingency basis so they don't paid unless the client saves money, usually as a % of the savings. A "win-win" for both.
This is usually an easy task considering there are several pieces of research from industry analyst that show that average mid-market companies overspend from 7-12% each year through billing errors, late fees, and carrier overages.
