I am not sure the term is all that new. Basically it is the practice of monitoring an application (and its related infrastructure components) to determine if it is meeting the needs of its users in terms of uptime and speed, and if not, taking steps to make it better.
In today's world of networked applications, the performance of the server an application is running on is just one piece. In order to determine what changes need to be made to increase performance a variety of techniques would be used. Depending on the technology stack an application uses, the collection of performance data can be straightforward. Many application frameworks collect performance data automatically and you just need to harvest and analyze that data.
Performance data available from the various infrastructure components (eg, the network, the server, the SAN) coupled with the application level metrics *can* paint a pretty clear picture of where attention needs to be focused. Sometimes, though, the picture can still be muddy, even with all that data.
