Find out how long has staff worked there? If there's huge turnover, what is causing that, and if there's loyalty and longevity then find out about that too. Does leadership recognise the talents and skills of the staff? Is it a learning environment where you can grow?
Bottom line you want to know if it's a healthy place to work. Speak to employees as much as leadership. Look for the unspoken word and whether they look stressed or pumped.

The most important question to ask is of yourself "Will this be a great place for me to work so I can't wait to jump into the deep end of each day?"

In addition to what Donna said (which are all really important pieces of information to gather), it's also good to ask a bit about the strategic direction and financial stability of the company. First, do some research on the financial stability of the company. Look at the company's history of growth and how it has expanded (or not) over the past five years. Also, get a good feel for the strategic direction of the organization. Where is it going moving forward? Who are the key customers and are they in stable businesses? Is there a plan for market expansion? Where? How? When?
You want to make sure that the company is going to be around in another 5 years which is why it's important to look into these things as well as what types of career development and growth opportunities there will be for you, as an employee

Ask the hiring manager "How do you know that I will become a successful employee if hired? If there is no answer, the lack of an answer is telling you something. If the answer makes no sense, the answer is telling you something.
