What are the strategies you use to turn SharePoint governance into an ongoing, active process?

SharePoint governance means more than creating a governance document and storing it in the document library. It should be a living, ongoing activity. What are some ways that you make governance an ongoing, active process at your organization?

I have worked with a framework for this for the last 6 years. When doing a governance implementation project we are very strict about defining the outcome as a "Governance Practice", which includes an organization and some well defined procedures and tools for stay on target - or in compliance, if you will.

It goes a bit into the framework which I have had some successes with, and the roles and responsibilities that are tied to this type of practice.
