What are your favorite exit interview questions to ask terminated employees?

I know all the standards, what could we change, how was the staff/mngt etc, what did you value. What I would like to know is who out there has gems that uncover truly valuable feedback for small business owners...

- What could we have done better as an employer?
- What could we have done better as a service provider to customers?
- Is there anything you think we should know about your experience as an employee?
- What could we do to make this a better place to work?
- Do you have any remaining/outstanding/open issues that I need to address?
