I am a huge fan of LiquidPlanner for online project management, including issue tracking (disclaimer: I am also a social media evangelist for LiquidPlanner, but I was first and always remain their biggest fan). I also used Fogbugz at a past company and found it useful and flexible for issue tracking, but tried to use it as a full Project Management tool and was disappointed.
Over at bime hq (http://bimeanalytics.com) we use Lighthouse bug tracker. It's cool because the solution we built (bime) also connects directly to Lighthouse, so we can use our own tool internally alongside it to produce the best results :-)
We're a SharePoint consulting firm and we've been using SharePoint itself for issue tracking. It's not a canned package, but platform features help a lot.
