It's one that I submitted.
I was working in telecommunications for IBM. Equipment was hooked up with cables having male and female connectors. Sometimes, an adapter was needed to change the gender of a connector, instead of buying a costly new cable.
A parts catalog at the time listed "Sex Changers" for such a purpose. It was a small adapter that would convert the male end of a cable to a female, and vice versa.
After purchasing one out of pocket, I submitted an expense report to local IBM people for reimbursement for a "Sex Changer". A month later, I was called on the carpet by IBM corporate who wanted an explanation. I'm sure they were planning on firing me. I showed them the catalog. They didn't know what to say for a few minutes. Later on, I got paid.
