What is the typical IT company staffing structure including on-site and off-shore team?
I have seen various staffing structures during my 12 years IT career. I would like to see a typical IT company staffing structure. It includes the designations, who will report to whom etc from top to bottom. Consider, IT company may have on-site and off-shore team.

I don't believe you can create a typical IT structure since businesses vary so dramatically. What works for a 5-person shop will not work for a 1000-person shop. Even among similar sized orgs, differences in corporate culture, accountability, compleity, and trust will change the staffing structure.
Rather than trying to shoehorn a structure into an existing org, it's better to evaluate the org and determine what's the best staffing and structure.
I agree with Kaiser, I have been with international, national, and regional consultant companies. I own my own as well. All were structured and ran very differently. Whether its how management is run, or even to something as simple as do you ever talk to the Director. The size of the company also plays a huge part in the culture of the company. I would take a step back and see what is best for your/their company.
