What items should a good hr policy entail?

Our company's HR policy is ancient and in desperate need of an update. I'm trying to revise it to meet the current needs of our company. What items / issues should a really good HR policy entail? What things are critical not to leave out?

What a great task! I think there are many great HR policies and some pretty bad ones. But the bad ones are never read or followed.

For me, great HR policies must have a purpose statement to begins with. Why do we have this policy? What is its function? If you can get that right, then it leads the person who it applies to. That purpose should be aligned to the purpose, values and mission of your whole organisation.

Secondly, the policy should apply to many people. If only a few people need it, and infrequently, it will be quickly forgotten.

Thirdly, it should have a built-in review date to keep it fresh. I know you will appreciate this last part. Send ancient HR policies to HR Musuem!

Paul's advice is pretty good.

I'd add that if you want people to read it, keep it simple, keep it short and try to cover anything but the legal basics in as little space as possible.
