The non-question, "How are you today?" as an opening to a telephone solicitation tops my list. I call it a non-question because the asker really doesn't give a damn about how I am today, or any day for that matter.
In the retail environment, "How can I help you" is tired and terribly overworked. Almost anything is better. If a salesperson can't come up with something more creative than that as on opening, they should find another career.
In the B2B sales environment I dislike the question, "What will it take to earn (or get) your business?" I'm always tempted to respond with, "Do your job properly. That will do it."
I think I'm getting grumpy in my advanced years. :-)
