As soon as the business can afford it you should get a managed server.
We have a large business with hosted services being accessed by clients. Our reputation with the customers revolves around having the fastest and most reliable access to these services possible. By having a dedicated server, then no-one else's problems can affect ours. By having the service managed it means we concentrate on the software we write, and we pay experts to maintain the server to the best possible extent, and to be awake at 2am in case of hardware failure. Reputation amongst the customers is everything.
It all depends on your usage and if you're having uptime issues. If your sites are going down or you're getting notifications from your webhost about overages, then it's probably time to upgrade your service.
I would say - be proactive.
You know the dangers of a shared solution: performance, downtime, insecurity/unreliability etc.
If your business needs the protection and security of a managed virtual server - you will also see the benefits of a firewall, upgrades/updates, instant scalability, dedicated resources and high performance.
Make sure,when you're ready to make the transition, you ask some serious questions about the offerings and services you are receiving. Go for dedicated resources (like a VPS) and managed services.

If you are using paid traffic platform like adwords, display media buys to generate traffic for your site then shared hosting is a surely not adequate. It depends on traffic and use of site. Moving from VPS to a dedicated server can be a little less clear-cut but recommend monitoring server usage before upgrading.
