At a local trade show last week, one person replaced his name badge by hanging his phone around his neck with a QR code showing on the screen to introduce himself.

At the Denver airport I saw an offer for Free Books (http://www.adrants.com/images/TDAFBBooks72.jpg) from a bank based in that city. I'm not sure what books has to do with banking, but I'm sure giving free stuff away has generated lots of positive PR for them. According to the article (http://www.adrants.com/2010/10/bank-offers-free-book-game-downloads-to.php) cross country air travelers are jumping at the chance to catch up on Sun Tzu's "The Art of War".

A marketing/promotions company customized one w/ Donald Duck. They use it to promote registrations for an upcoming open house

They are becoming more popular in Real Estate - either on brochures to go to a website with various listings, or on a "for sale" sign directing viewers to the photos of the house and other information online..
I've seen them used in mobile strategies where the QR code drives people to view a video or image, collects some data, puts you in the mobile subscription database, and enters you into a drawing - all in one fell swoop.
The other day I was shopping for clothes and the clothing tag had a QR code that directed people to the online store to shop for the item in case I needed a bigger/smaller size, another color, etc.
I would LOVE to see QR codes on games, toys, books, etc. for kids that link back to the Parent's Choice Foundation ranking system - parents can quickly determine if the item they are about to buy falls in line with the quality/content they wish for their kids to be exposed to.
Best Buy does this very well in the store - scan the QR code on the price tag and it takes you to their online rating system. You can immediately see reviews for the product while you are in the store!
Like many folks, I use a QR code on my business cards that drives people to a "So glad we met" landing page on my personal website, which then asks them to subscribe to the blog or newsletter.
I used to have the QR code point to a .vcf contact card that people could automatically download and load me into their contact list on their phone, but that app stopped working recently.
Coors Light used the tagging system (like QR codes) in their Superbowl promotion this past fall. It was pretty cool... Inside of the case of beer was the tag for you to scan. It took you to a website where you could enter to win free tickets to the superbowl. Purchase required.
It was a great way to get people to stay engaged with the brand post-purchase, and even after disposing of the product - something more marketers need to evaluated as part of their marketing efforts.
We use a QR code to send out our eNewsletter to our open source, SIPfoundry community user forum, and as others stated on the back of our business cards with links to our whitepapers and case studies.
Recently I attended the Enterprise Connect Expo it was placed on all Banners for downloading the schedule of events for the week. I referred to this multiple times through out the week, very useful and no cumbersome paper schedules to keep track of.
I'm not sure if this counts as 'most creative', but I was very impressed when I landed in Tokyo for a visit a few years ago to receive a QR sticker in my passport instead of the traditional stamp at Customs & Immigration (this was right after both my index fingers had been digitally scanned).
I use it my corporate logo - I get either "great idea" or "what's that?" which is just a good.
I used PingTag (http://pingtags.com/) to create an QR code that shows my LinkedIn profile. I add that QR code to event name badges, which becomes an "ice breaker." People either ask me what that "design" is or scan it. Either way, it initiates word-of-mouth marketing for me.

Last week I was having lunch at a local mexican restaurant with a colleague and we noticed there was a QR code on the menu. We each got out our iPhones and spent about 5 minutes trying to scan it ... we never got it to work. It would have been great marketing if we got it to work, especially if there were a coupon as a result.

The human QR codes and the one done for coke were my personal favourites.
The most creative QR code use I've seen was as the answer to a joke. The answer was hidden in the QR code. You can find more unusual QR codes at www.barcode.com.
How about T shirts with QR codes for outing away from school there could be a hand held scanner for making sure everyone in back on the bus. School card QR code for young children getting on and off the correct bus. Are all the children off at the end of the route? Many time during school children fall asleep and get left on buses.
