What's wrong with project management?

A number of things;
- Lack of OCM (see my Column on ITSMPortal which discusses this in a different context - http://bit.ly/cYFhry)
- Oversold, under-delivered technology (Software sales rep. did a good job selling how quick system can be implemented/changed)
- Changing requirements (result of the following)
- Misunderstood Architecture (why, what) - IT organizations only focus on the how and with what and fail to answer the why and what...
- Project Managers inability to create waves due to career path (have always felt contracted PM's did better than internal, because they have a project to worry about and can upset the apple cart within the organization)
To name a few. I am sure there are MORE
Do you have executive/stakeholder buy-in? Are THEY following the principles of project management? If not, there's one good answer.
Why do many projects still disppoint?
(a) Because they are too focused on timescale and budget.
(b) They are managed as implementations rather than investments.
Very often, the most senior project manager is measured and rewarded based on inputs, activities and outputs (e.g. project costs, process changes, organizational changes, technology deployments, documentation) rather than business outcomes (e.g. benefits delivery, architectural contribution, +/- impact on future operating costs).
Also, projects that appear to be a attractive proposition in their Investment Proposal/Business Case are not constantly reviewed to ensure an early stop loss if the original hyopthesis turns out to be flawed.
