Try Nolo Press (http://www.nolo.com/). They publish many books on legal information and forms. Although, I would urge you to have an attorney review the forms and insurance requirements before using a generic template. Also you should require the lessee to obtain property and liability insurance and name you as insured.

To Mike - I still recommend an attorney's review of any agreement leasing heavy machinery. You want to make sure the lessor is protected for any damage to property or for personal injury caused by the lessee while using the machinery.
I was hoping to dodge the attorney route, but if you think it's in my best interest, I'll check it out. Thanks for commenting and providing links.

Mike, even if you don't go the attorney route, make sure you require the lessee to obtain and provide proof of insurance to you with an insurance certificate. He should also name you as additional insured on the certificate. Your insurance agent may be able to provide insight on covering your risks.
