I would recommend Salesforce.com, Act, Goldmine, or SugarCRM. Each has its pros and cons. I would look into each of them and see which one you think makes the most sense for your company. Hope that helps!

I think Salesforce.com and SugarCRM are your best bets, but you might also look into Maximiser or even FreeCRM.com.

For overall CRM functionality Zoho CRM is GREAT for small businesses, as well as Sugar CRM (Express) or Salesforce.com's Group Edition.

If you find that those solutions are too "sales-ey" for your needs, then consider looking into a CRM that is oriented towards the healthcare industry. Microsoft has a version of their Dynamics CRM that is oriented towards the healthcare industry and gets great reviews. Also Salesforce offers a healthcare industry CRM, and CPM Marketing and Pegasystems both have strong healthcare oriented CRM's.

Oracle is sometimes characterized as only having CRM for large organizations. This is not accurate. They have done a great job building out industry editions of their CRM On Demand offering which would help a doctor/dentist office hit the ground running with a lot of pre-built industry content. They cost less then salesforce.com and have much more functionality than ACT, Goldmine, Sugar or Zoho.
