The question is too broad for a succinct answer.
I suggest the question is refined to focus on the type of agency that you're interested in, e.g. creative, media buying, events, digital, analytics, shopper marketing, and the list goes on...

I tend to agree with Fred. The BtoB agency landscape has become highly specialized. Given the influx of emerging marketing technologies and the integrated nature that most programs are taking on, some interesting areas of specialization are emerging and I'd love to see discussion on top vendors from this group specific to key business challenges BtoB marketers are facing.
In cased you missed it, BtoB Magazine released their 2012 Marketers Resource Guide two weeks ago. While I don't necessarily find the structure useful, it is arguably the definitive BtoB agency guide and is a steal at $12.95. http://www.btobonline.com/section/pdfsale01
I would love to see a version of the BtoB Magazine resource guide that's ranked by customer ratings and/or social followers. Show me the agencies by use (creative, strategy, lead generation, consulting, etc.) that have the best reputations, deliver the best results, and generate the best content for their followers.
I'd love to see that ranking if it exists. If not, maybe Focus could create it for us...
Regardless of criteria, the top five would have to include MLT Creative. Those guys are awesome!
