I used LegalZoom.com. They were awesome. I had the same questions and they will know what's best for your situation. With LLC you have a lot more protection, but I will leave it up to them to explain to you.

NOLO has a great book available at most bookstores that lays out the plus and minus of C Corp, LLC, etc. It's a good investment, allows you to speak with a professional with some good education behind you to ask the really good questions.
I highly recommend it.

And as for getting forms, you can get them directly from your states Attorney General's office -- two ways to check. Look for Business resources on your state's website or go to your Attorney Generals' website.
