I would say it comes from the top. The leader of the organization sets the tone for the workplace and it gets filtered down from there. However, individual departments may stand out due to their leadership but it does not usually "infect" the rest of the workplace.
Also as a general rule (not one I particular like) is that we manage people based on their weaknesses or areas of development instead of focusing on their strengths. If all organizations were to focus on their people's strengths and they have the right combination of strengths, each organization would flourish.
