What are the most common survey design mistakes researchers should avoid?

I agree with David's answer "D. Providing answer sets that are not comprehensive and
mutually exclusive." Put another way, the most common mistake is not allowing respondents to answer they they like. Sometimes there are obvious holes or bias in the answer choices.

That depends on your budget. I like Cvent as a tool, full disclosure I blog for them, Vision Critical's Firefly is also nice. These are both in the mid-price range. On the lower-end I would try Question Pro. more
